The form allows you to provide the relevant clinical information and upload any supporting documentation, such as referral letters, investigation results or previous correspondence.
Once submitted, the information is reviewed by our team and used to create or update the patient’s clinical record, helping ensure that our clinicians have the appropriate background information before the appointment.
Please ensure that the patient is aware of the referral and has consented to their information being shared with us.
