Long-Term Health

Take control of your health for the long term.

Good healthcare shouldn’t only respond when something goes wrong.

If you’re living with diabetes, arthritis, a heart condition or several ongoing health problems, it can be difficult to see how everything fits together. And if you’re currently well, knowing which preventative steps genuinely matter can be just as difficult.

Over six months, you’ll work with the same GP to understand where your health is now, identify what matters most, develop a personalised Health Plan and measure the progress you make together.

A different approach to long-term health

Your health is more than a collection of diagnoses and test results. Living well with a long-term condition involves much more than responding to individual symptoms. Healthcare can easily become fragmented.

One appointment deals with your blood pressure. Another reviews your diabetes. Someone else treats your arthritis. Test results arrive at different times and recommendations can accumulate without anyone stopping to ask which things matter most to you.

Our Long-Term Health service starts from the opposite direction.

Your conditions, medications, lifestyle, physical health, family history and personal priorities all interact. Managing them well means understanding those connections and deciding where your attention will make the greatest difference.

Our Six-Month Health Plan creates dedicated time to do that properly. The objective isn’t to medicalise your life. It’s to help you make better decisions about your health.

Why choose our Long-Term Health service?

See the whole picture

We review your complete health rather than managing each condition in isolation.

Avoid unnecessary testing

We’ll use recent investigations wherever they’re suitable and only recommend additional tests where the results could usefully inform your care.

Build a relationship with your doctor

The same GP works with you throughout your six-month plan, developing a deeper understanding of your health, circumstances and priorities.

Who is the Health Plan for?

Diabetes. Arthritis. Heart disease. Blood pressure. Medication. Family history. Exercise. Sleep. Weight… Your health doesn’t experience these things separately, so neither should your healthcare.

Living well with diabetes

Diabetes affects much more than your blood sugar.

If you’ve already been diagnosed with diabetes, we’ll review how well your current treatment is working, identify opportunities for improvement and help you understand the different things you can do to protect your future health.

Where appropriate, you can add our Diabetes Investigations package to ensure the measurements needed to inform your plan are up to date.

Maintaining movement with arthritis

Living with arthritis isn’t simply about controlling pain. Maintaining movement, strength, independence and the ability to keep doing the things that matter to you can be just as important.

We’ll review how arthritis is affecting your life, your current treatment and medication, and what could help you maintain or improve your function.

Our optional Joint Health package includes an assessment with our physiotherapy partner to help develop the movement and rehabilitation aspects of your plan.

Protecting your cardiovascular health

If you’re living with a heart condition, high blood pressure or another diagnosed cardiovascular problem, we’ll help you understand the different factors affecting your health and bring them together into one coordinated plan.

Our Cardiovascular Investigations package can be added where relevant investigations aren’t already up to date.

Staying well before illness develops

You don’t need to wait until you become unwell to invest in your health.

We don’t believe preventative healthcare should mean carrying out dozens of tests simply because they’re available. Instead, we’ll consider your medical and family history, lifestyle and individual risks and recommend investigations where knowing the answer can genuinely help you make better decisions about your health.

Your Six-Month Health Plan

Understand where you are. Decide where you’re going. Measure the difference.

Your Health Plan combines comprehensive medical assessment with six months of continuity and structured review.

Step 1

Before your first appointment

We’ll send you a detailed health questionnaire to complete in advance.

This gives your doctor the opportunity to understand your medical history, existing conditions, medications, lifestyle, recent investigations and what you most want to achieve before you meet.

If you’ve recently had blood tests, scans or other investigations elsewhere, we’ll ask you to share the results where possible. We don’t repeat investigations unnecessarily.

Step 2

Comprehensive Health Assessment

Your first appointment is dedicated to understanding your health as a whole.

We’ll review your diagnosed conditions, medication, previous investigations, specialist care, lifestyle, relevant risks and preventative healthcare alongside the things that matter most to you.

Together, we’ll identify your priorities and develop your personalised six-month Health Plan.

Step 3

Month 3 Progress
Review

Three months gives you time to put your plan into practice and for many meaningful changes to begin showing results.

We’ll review your progress, discuss what’s working and what isn’t, consider relevant measurements or investigations and adjust your plan where necessary.

Step 4

Month 6 Health
Plan Review

At six months, we’ll bring everything together.

We’ll review the progress you’ve made, assess the measures that matter to your health and agree what happens next.

There is no automatic programme or unnecessary series of appointments after this point. Your doctor will simply agree the ongoing monitoring and care that’s right for you, which then continues through our normal GP service.

For £499, you get:

✓ Detailed pre-appointment health questionnaire
✓ 60-minute comprehensive Health Assessment
✓ Personalised six-month Health Plan
✓ Review of your conditions, medication, existing investigations, specialist care, lifestyle and preventative health
✓ 30-minute three-month progress review
✓ 30-minute six-month Health Plan review
✓ Continuity with the same GP throughout the programme
✓ Coordination of your wider care
✓ Transition into appropriate ongoing GP care

The same doctor throughout

Continuity is at the heart of the Health Plan.

You’ll have the same GP leading your care throughout the six-month programme, including your initial assessment and scheduled reviews. If you need an additional GP appointment relating to your Health Plan during this period, we’ll arrange this with the same doctor. Additional appointments are charged at our normal consultation rates.

Very occasionally, circumstances outside our control may mean your doctor can no longer continue your care. If that happens, we’ll arrange a full clinical handover to another suitable GP so that the knowledge and progress built through your Health Plan aren’t lost.

Investigations that fit your health

There isn’t one standard panel of tests that everyone needs. That’s why investigations aren’t automatically included in the core Health Plan. Instead, you can add the relevant investigation package if you need it, from the below choices. If none of these suit your needs, your doctor will suggest appropriate investigations during your initial consultation.

Diabetes Investigations

For patients with diagnosed diabetes who don’t already have sufficiently recent monitoring results.

£379

✓ HbA1c
✓ Full lipid profile
✓ Urea & electrolytes
✓ Urine ACR

Joint Health Assessments

There are no standard investigations that everyone with arthritis needs. Your doctor will review any previous imaging and may recommend an updated X-ray or other investigation if existing results are old or no longer sufficient to guide your care.

£85

Add an initial assessment with our physiotherapy partner, focusing on movement, strength, pain and function, with recommendations incorporated into your wider Health Plan.

Cardiovascular Investigations

For patients with an existing heart or cardiovascular condition who need an up-to-date clinical baseline.

£589

✓ Full lipid profile

Preventative Investigations

For patients taking a proactive approach who would benefit from establishing an appropriate baseline.

Common Questions

No. The Health Plan supports both people living with diagnosed long-term conditions and people who want to take a proactive approach to maintaining their health.

Our Diabetes, Arthritis/Joint Health and Cardiovascular pathways are designed for patients with existing diagnoses. If you are currently well or want to understand potential health risks, the Preventative Health pathway may be more appropriate.

Your doctor will consider how your different conditions, medications and priorities interact and develop one integrated Health Plan around you.

No. Some patients join because they want better support managing an existing condition. Others are currently well and want to understand how they can protect their future health.

The approach is the same: understand where you are, decide what matters and focus your effort where it can make a meaningful difference.
